At Infinity Health, our Chronic Pain Management Program offers timely access to an interdisciplinary rehabilitation team to support meaningful, long-term recovery. We integrate Occupational Therapy and Clinical Counselling to deliver coordinated, whole-person care tailored to your needs. Clinical Counselling is currently available virtually, with in-person options coming soon.

Collaborative, whole-person care

Chronic pain affects more than just the body; it can influence mood, energy, cognition, sleep, daily routines, work roles, relationships and quality of life. Our team works collaboratively to address the physical, cognitive, emotional and functional impacts of chronic pain. Together, we develop a personalized plan to help you build skills, increase confidence and safely return to the activities you need and want to do.

  • Ergonomic assessment and environmental modifications
  • Evidence-based strategies for relaxation, stress management, and fatigue management strategies
  • Guided exposure-based strategies to movement
  • Support engagement in daily activities through behavioural activation, scheduling, and goal-setting
  • Supportive, trauma-informed care to help manage the impact of mood, stress and emotional wellbeing on daily functioning
  • Sleep hygiene education and sleep strategies
  • Cognitive remediation or compensatory strategies to address “brain fog”
  • Empowering clients with practical tools for independent symptom management
  • Address persistent pain conditions, including Complex Regional Pain Syndrome (CRPS)

  • Emotional regulation and distress tolerance skills
  • Trauma-informed care for conditions such as depression, anxiety, and PTSD
  • Identify barriers such as fear-avoidance, catastrophizing, and other pain-related barriers
  • Strategies based on Cognitive Behavioural Therapy (CBT) and Acceptance and Commitment Therapy (ACT)
  • Support for mood stabilization, coping skills and stress management
  • Address motivational barriers and support behavioural change

Contact us to speak with our Intake Coordinator, who will schedule initial assessments with our Occupational Therapist and Registered Clinical Counsellor.

Before your appointments, we’ll send short online forms to help us understand your current symptoms and concerns.

Attend your assessments
Each assessment is completed separately. Our team will then prepare a detailed report outlining the findings and a personalized treatment plan.Begin your rehabilitation
