Being discharged from a hospital can often be a stressful experience, especially if you have been impacted by an illness or injury. Our occupational therapists can help you to ensure that the transition home is safe and ensure that there is continuity in care.
Hospital Discharge Service Overview
The Infinity Health team can work with you to create a safe hospital discharge plan to prepare for your return to your home after a hospital stay. The hospital discharge plan managed by your occupational therapist can include:
- An assessment of any physical and cognitive barriers that may impact your ability to return home safely.
- An assessment of your home to determine any equipment, assistive devices and environmental modifications required to ensure your own safety in your home.
Your occupational therapist will work with you to:
- Reduce functional difficulties in your home by providing you with the tools you need to participate in your activities of daily living. For example, providing assistive devices training, mobility aid training, and provide training to care aides to meet your individual care needs.
- Communicate with your doctor to ensure that all your needs are met when you return home.
- Complete a follow-up visit at your home to ensure that you are provided with the correct home management tools.
- Provide an assessment to determine if you need services at home.
- Coordinate any ongoing changes in functional status that may impact your ability to complete home-related tasks.
